Preparing for Your Admission
Entering the hospital can be a stressful event for patients and their families. Our healthcare team is here for you. It is our desire that you and your family are well informed and prepared for a successful and safe hospital experience. Our goal is to not just meet, but to exceed your expectations from the moment you arrive at Rex and throughout your stay.

Pre-registration Information
Please complete the pre-registration form at least five days before your date of service. This will allow Rex to to ensure your account information is current and your service can be provided without delay on the day of your visit. If you have changed your name, address or insurance coverage since your last visit, you must pre-register. If you pre-register by phone or fax, your registration will be much quicker on the date of your procedure or surgery.

3. At the time of check-in, please be sure to bring all insurance cards, your legal ID (driver's license or other ID) and any co-pay or co-insurance for the service to be provided. If we do not receive a pre-registration form from you prior to your visit, we will place a telephone call to your home a few days before your visit. Please have your insurance card available to help with pre-registration.

What to Bring
Please bring the following items on the day of your pre-surgery planning appointment and on the day of your procedure:
- Health insurance information
- A valid photo ID
- Co-Payment/Payment
- Completed Personal History and Health Information Form
- The most recent labs or diagnostic test results such as MRI, x-rays, EKG and lab work
- Physician orders and other paperwork provided by your physician's office
- Advanced Medical Directives Information
- A list of any questions you may have regarding your procedure or your stay at Rex
